Are Medical and Surgical Treatments Successful
Even in the situation of couple’s normal and regular relationship, pregnancy probability is once in a one month process and its rate is not more than %5-10. Because of that reason, pregnancy’s delay till one year is accepted as normal. In slight abnormalities for one or both of the couples, pregnancy delays a bit more. It is hard to forecast the treatments’ efficiency. If the abnormality’s reason can not be understood wholly, the treatment methods are used as trial and error method. If it is taken into consideration that sperm production time in men is minimum three months, we cannot determine whether a treatment method is successful or not before three months. Moreover, the same situation is discussed in women.
In some diseases, doctor holds a view that the patient would not have a child. Sometimes there is a little bit inefficiency and in this situation even if the couples are not treated there is pregnancy probability. However, a treatment is still applied. Eventually, is the pregnancy success of the treatment? Or will the pregnancy occur without the treatment? It is hard to answer this question. However, there is tendency of accepting the result as the success of the treatment.
If the infertility reason is determined, treatment method and success probability is obvious. Various methods are applied because of that reason. But, it is not probable to determine the reason every time. This situation is the consequence of technical impossibilities and insufficient survey. Because, it is obvious that there is problem with respect to not having pregnancy. To mention about the medical and surgical treatment’s success possibility, it is right to tell that infertility reason should be determined.

Who should be applied to?
Men infertility should be examined and treated by urology and women infertility should be examined and treated by gynecological disease and birth clinics and experts. However, both science fields are so vast because of their scope. Because of that reason both clinics and experts who deal with the infertility have more experience about the subject. In lots of Western countries, men and women infertility became a separate science field. Studies in these subjects are keeping going in Turkey. Andrology science gathers the laboratories in which studies about sperm are made besides men infertility and sexual disorder clinic. Furthermore, there are women infertility clinics. The truth is to determine the men and women infertility in separate centers, but both centers should be in coordination for the treatment. These kinds of studies have risen in late years.

There are various reasons of infertility. The reason of not having baby is not only depend on the women. There is also probability of men infertility. Although it is not common, both wife and husband can be infertile in some situations. Whatever reason they have, statistics show that %17 of the married couples are complainant of infertility.
The couples who have not got baby in the early years of the marriage cannot resist asking themselves: “I wonder whether the reason is me?” There are some men who divorce from his wife without applying to a doctor. And these men refuse to apply to a doctor with the fear of “What if the problem is mine?” Like all diseases, searching cure in infertility is a sense of logic.
Infertility Ranges:
Temporary infertility (enfertility):
It is the infertility which occurs as a result of not having live semen cell (sperm) which ensures the insemination. In this type infertility, miscarriage can be observed frequently.
Primary Infertility:
It is an infertility type which is seen in newly married couples. Its reason is psychologically more than organically. After the couples get accustomed to each other, it is determined that %33 in the first three months, %70 in one year, %80 in two years and %90 after two years have children. As taking into consideration of this situation, the newly married couples should not abandon themselves to despair and should wait at least two years. If the infertility goes on after two years, it is beneficial to apply to a doctor for them.
Secondary Infertility:
It is an infertility which is seen after the couples have one or two children. Its reason generally is malnutrition and disordered life standards. Inflamed wounds which occur in oophoron of women, zygote channel and uterus are the frequent infertility reasons. Exposing to a scrotum inflammation of men who experienced mumps is also a reason of infertility.
Permanent Infertility (sterility):
It is an infertility which has no chance to be cured for now and existing medical knowledge is inefficient for both in women and men.

What Kind of complain Myoma can cause?
Although most of the myomas do not show indication, they create some complain in %25 of the occasions. The most seen ones are extreme and abnormal vaginal bleeding, pain and puffiness on belly.
1. Menstruation disorder:
Menstruation bleedings are more than normal in %30 of the women who have myomas. Submucosa and intramural type myomas are the ones which cause bleeding. The mass pushes the endometrium tissue by growing and consequently this tissue’s acreage grows.
While the bleeding time does not change at the beginning, only the lost amount of blood changes. Then the time starts to change slowly. This extreme blood losing then causes anemia. Some myoma types can cause the interval bleeding with the extreme bleeding. The most important finding which makes it a must to apply a doctor for women who have myomas is the bleeding disorder.
Bleedings can be so extreme that person must change the pad once an hour. A woman who experiences this bleedings cannot want to do daily activities. She can avoid from going to job and social phobias can occur. So, myoma is a disease which affects the women’s social life.
Bleeding’s probable causes in myoma:
• Endometrium surface’s growth
• Growing of being ribbed in uterus
• Endometrial hyperplasia which is seen at the rate of %50
• Little vessel mouth’s not being closure because of the not effectiveness of uterus convulsion
• Ulcer comprising in the endometrium tissue around the Submucosa myoma
2. Pain:
Pain in myoma is not a common symptom. Generally it is cramp style during the menstruation bleeding. Sudden pain appearing in women who have painless menstruation bleeding for long years must remind the myoma in diagnosis.
Experimental studies think that pain mechanism which is seen with the myomas similar to birth pangs. Myoma core pretends to be a foreign substance and the uterus strains to get it out.
3. Puffiness on Belly:
Myoma pushes the other organs by growing and this situation can cause lots of illnesses. It can cause frequent pee if it forces the bladder and can cause constipation if it forces the rectum. Again, gas problem can be seen according to the forces in intestine.
4. Infertility:
Myomas can make it hard to be pregnant for women or to carry the pregnancy for uterus. While the myoma continues to grow, endometrium layer on it stretches and its bleeding becomes hard. In this situation even if the pregnancy product settles in the uterus, it is not possible to be bloody adequately and it can cause suffering a miscarriage. The other disadvantage waiting for the product of the pregnancy is not to be a vacancy for the baby. The most probable end waiting for the pregnancy is miscarriage or premature birth.
The other problem in the situations, in which myoma and pregnancy is together, is uterus’ inadequate strains during the birth. The baby cannot enter the birth channel properly and most probable cesarean should be needed in these patients. When the myoma closes the birth channel, cesarean is the only way to birth. Extreme bleeding can be seen after birth because of the ineffectiveness of birth strains.
Generally, myomas do not cause any problem for both pregnancy and continuing of the pregnancy. However, if a problem occurs, this will be a serious problem. To mention about myoma’s infertility causing, there should not be other reason to explain the infertility. So, the whole survey which is done in the survey of infertility should be done in infertile patients.
